Dental Professionals whose record still shows a license status of Active, CE Request, CE Extension or Renewal in Process is active and may continue to practice pending the review of their renewal application by the Board or its designated authority.

For insurance credentialing or to verify a license - all information is from the Board's database, and therefore, the data can be used for primary source verification of an Arizona license/certificate holder. While we believe the information to be reliable, human or mechanical error remains a possibility, as does delays in posting or updating information.   

Pending, dismissed and terminated complaints are not available to the public pursuant to A.R.S. § 32-1207(A)(3).

Letters of Concern are not available on the website, but are available upon written request for up to five years from the date of action pursuant to A.R.S. § 32-1207(A)(3)(b). Please call the Board office at 602.242.1492 to request copies.

Non-Disciplinary Action, taken before 1/1/2018, is not available on the website, but is available upon written request for up to five years from the date of action pursuant A.R.S. 32-1207(A)(3)(b).
